Use concussion grenades a lot.  Don't go around killing people, as
this is pointless because all they do is respawn and you run out of
ammo.  Instead, concuss them and run past.  They will be useless in
defense or offense, unable to do anything.  Much better than killing
them then watching them respawn and shoot you.  Also, when you run
back up with the flag (on a 2fort5 style map) they're all concussed so
you have no problems getting out.  You should never just throw a
grenade, as the enemy will see it and run.  Instead, you should prime
it for a while before throwing it so it blows up in their face.  If
you get concussed yourself, don't try to turn around or aim up and
down.  All it will do is throw you off.  Instead you should only
strafe.  This way at least you can go somewhere.  Concussion is
extremely powerful in disrupting an enemy's team.  I've seen people
suicide because they've been concussed... just goes to show how
annoying it is...

I agree as far as it goes on concussing them to confuse them instead of killing them with the conc grenade. But I don't feel you should always run past. If the enemy is by himself, then you should finish him off with your nailgun. If there are 2 or more enemies, then it might be safer to run...
It would also be wise to move back from a demoman after concussing him, as he will shoot wildly around with his grenades, possibly killing you.

In response to OZ...

It depends on the type of enemy you conc'ed. If it's a demo well, I think
it's best to kill him.(or maim him seriously enuff so that little threat is
posed on your way out) But if it's a soldier or HWGUY for instance, then
it's best to run by even if it's a single player. In and out should take
you less time than it takes for the soldier to feel better. And depends on
LPB status of your enemy and if there's a med near-by. So it's basically up
to you when conc'ing. See if you can take a quick look through the place
you want to go through. If it's the ramp room, then through a Conc gren
below the places where the enemy are most likely to stand. (cons go through
floors and walls) Then give 'er hell and grab that flag!
